Type 1- Individual ‘people’

• Each Individual  has only one number and that number is theirs for life. 

• This includes but is not limited to physicians, podiatrists, dentists, optometrists, physical therapists, occupational therapists, certified registered nurse anesthetists, nurse practitioners, physician assistants, chiropractors, etc.

Type 1- Individual Providers-Sole Proprietorships

Type 1 providers  in practice by themselves or incorporated as sole proprietorships -only need an individual NPI.  This applies to physicians, podiatrists, dentists, etc.
