Applying for your NPI | |
Providers at one time could complete either a paper form or an on-line application via the National Plan & Provider Enumerator System (NPPES) Web site. Only electronic requests are now being processed. The administrator of the NPPES will process the application and assign a unique NPI number. After the number is assigned, the provider will be notified. Electronic Requests NPPES User ID When you register online you will create an user identification (ID) and password. The user ID should be recorded and stored in a secure area. If the professional moves or needs to request additional NPI’s you will be asked for your user ID to access the information.
