claims are unique and include:
- Boxes around the headers for each amount
- Line by line breakdowns
- Codes billed by line item and then, if applicable,
the code(s) bundled into them
- Specific error messages

Activate Member Choice Fund payments
When a Activate member elects to use his or
her Member Choice Fund to pay medical deductibles
or coinsurance, the payment will be noted with
a new explanation code of ZMO, Member Choice
Account payment.
