BlueValue

Our BlueValue medical product is available to employer groups located in the Portland metropolitan area with at least 100 employees. It utilizes the Oregon Select Networks, our limited provider networks centered on several core health care delivery systems in the Portland metropolitan area.

BlueValue is designed to encourage member awareness of the cost of health care and seek higher value health care for their chronic conditions.

The tiered benefit structure offers promotion of preventive care and active management of chronic conditions with different value levels:

Value level 1
The deductible is waived for preventive care services, specific generic prescription medications and specific outpatient laboratory services related to the following conditions:

• Asthma
  
• Depression
  
• Diabetes mellitus
  
• Diabetic eye examinations
• Coronary artery disease
  
• Congestive heart failure
  
• Chronic obstructive pulmonary disease

Value level 2
The following types of services are covered after the annual deductible is met:

• Maternity
  
• Emergency room
  
• Hospital and facility services
  
• Other prescription medications
  
• Office visits and other professional services
• Specific surgical services (additional copayment applies)
  
• Complex outpatient imaging (additional copayment applies)
• The coinsurance maximum applies to both value level 1 and value level 2 services.

Provider networks
At enrollment, members will choose one of the following Oregon Select Networks for Category 1 benefits:

• Oregon Select Tuality
  
• Oregon Select Providence
  
• Oregon Select Legacy Health
• Oregon Select Adventist Health
• Oregon Select Oregon Health & Science University (OHSU)

Members can seek services from any provider. However, the benefits are designed to encourage members to seek services from their chosen Oregon Select Network providers. A member’s benefit level is determined by his or her choice of provider and the services received.
