| The Participating (PAR) network is
used for traditional indemnity plans in Oregon and
in Clark, Skamania, and Klickitat counties in southwest
Washington. The PAR network is offered by Regence
BCBSO, Regence Life and Health, Regence BCBSO affiliates,
other Blue Cross or Blue Shield Plans (Blue Plans)
through The BlueCard® Program, and Regence BCBSO
subsidiaries. |

Plan (PPP, PPO) |
| The Preferred Provider Plan (PPP)
network is used for preferred plans in Oregon and
southwest Washington. The PPP network is offered
by Regence BCBSO, Regence Life andHealth, Regence
BCBSO affiliates, Blue Plans through The BlueCard
Program, and Regence BCBSO subsidiaries. |
| Access |
| The Access network is used for preferred
plans offered by Regence BCBSO in Multnomah, Washington,
Clackamas, and Columbia counties in Oregon, and
Clark County, Washington. The Access network is by
invitation only, and is maintained based on performance
and utlitization standards. This network is offered
by Regence BCBSO and other Blue Plans through The
BlueCard Program |

Choice Sixty-Five |
| Preferred Choice Sixty-Five is a Medicare
managed care plan which combines
the cost-saving features of managed care with the
flexibility of traditional Medicare coverage. Preferred
Choice Sixty-Five provides all the benefits available
from Medicare Parts A and B, and Medicare supplemental
plans. |
| Regence MedAdvantage |
Our MedAdvantage product uses the
Regence BCBSO MedAdvantage network for in-network
services with no referral requirements to access
specialty care. Regence MedAdvantage provides all Part
A and B benefits with additional covered services
in one plan. Most professional services are covered at 100% after a $5 copayment for in-network providers.s
We are actively contracting with physicians, health care professionals and facilities for our Regence MedAdvantage network. |

Complementary and Alternative Medicine (CAM) includes
the following provider types:
- Acupuncturists
- Chiropractic Physicians
- Massage Therapists
- Naturopathic Physicians

The PAR network for CAM providers applies to members enrolled in the Federal Employee Plan, other Blue Cross or Blue Shield plans through The BlueCard Program and members enrolled in Innova and Engage plans. This network is not closed and we are actively contracting with CAM providers to be included on our PAR network. |
