What is the Regence AdviCare Program?
The Regence AdviCare Program is a proactive, and physician-
and patient-focused education and support system for
people living with chronic conditions. When we asked
physicians how we could best support them, the answers
were to encourage patients to take a more active and
responsible role in self-care and to emphasize the
importance of compliance with physician recommendations
and treatment plans.
The Regence AdviCare Program encourages participants to comply with their physician’s treatment plan and offers a broad array of easy-to-use tools and practical resources that are designed to make the delivery of quality care easier. The primary goals of the program are:
- To help participants living with chronic conditions better manage their health, reduce related health problems, and improve quality of life.
- To achieve consistent, quality care by using nationally recognized clinical guidelines.
- To support and reinforce the physician’s treatment plan.
Program design
The program was designed with the input of respected experts in the field to provide specialized services to people with chronic conditions. By using nationally recognized clinical practice guidelines and best practices as the foundation of the program, we aim to work in cooperation with participating physicians to achieve consistent, quality care levels and improved outcomes.
The Regence AdviCare Program team, led by a team of health care professionals, including registered nurses and dietitians, provides support to program participants and reinforces their physician’s treatment plan. Primary care physicians may receive patient-specific reports, condition-specific newsletters, and care guides, which provide salient points of best practices as defined by nationally recognized, evidence-based clinical guidelines. Examples of participant materials include condition-specific workbooks and welcome letters, newsletters, standards of care reminders, and self-care goals established with program health care professionals.
Employers can choose from packages that include specific
conditions, based on the analysis of their population. Conditions
that may be included are:
